LASIK is one of the surgeries that treat refractive errors of the eye. Lasik has quickly become the procedure of choice for most patients because they recover quickly and have fewer side effects and complications than with other methods of vision correction. Over the years, Lasik has come a long way. With advance in technology, vision correction has now become an easy and extremely safe procedure. Femtosecond Lasik or Bladeless Lasik is the latest technology introduced for patients suffering from Myopia (near sightedness), Hyperopia (far sightedness) or Astigmatism.

But, before you choose Bladeless LASIK or any other form of vision-correcting surgery, there are several criteria to consider. Firstly, are you a good candidate for the surgery? Which of several procedures is best for you? What are the risks? We try to help you find the answers for the same here.

The FEMTOSECOND TECHNOLOGY

LASIK stands for Laser In-Situ Keratomileusis. It is a form of laser eye surgery that can correct a wide range of vision imperfections. Bladeless LASIK or All-Laser Lasik or Femtosecond Lasik is a procedure that permanently changes the shape of the cornea, the clear covering of the front of the eye. It differs from other LASIK surgeries because the corneal flap is created using a laser as opposed to a manual blade. With this bladeless Laser Lasik technology, people with thin corneas who once were unsuitable for LASIK may now be candidates. Its precision and ability consistently produces corneal flaps thin enough to help treat those patients who were previously not eligible candidates.

The WaveLight® Refractive Suite technology by Alcon that we use for Bladeless Lasik is supremely efficient and fast. The Femtosecond Lasik surgical procedure involves creating corneal flap without a blade ever touching eye! This truly integrated combination uses the world’s fastest excimer laser available—with the most sophisticated technological laser vision advancements—to ensure accuracy, safety, and consistent refractive outcomes.

Bladeless Lasik uses 2 types of Laser. First the femtosecond laser creates a hinged flap in the cornea, which is lifted so that the eye surface beneath can be accessed. This laser operates at exceptionally high speed and divides the tissue on a molecular level in a manner so that it does not transfer heat or damage to surrounding tissues.

The second laser used in Bladeless LASIK is the excimer laser, the tool which performs the actually surgery on the exposed eye surface. The EXCIMER LASER is a “cool” laser, and does not generate heat in the cornea. This laser removes tissue in a specific pattern in order to permanently alter the shape of the cornea. Once the re-shaping is complete, the Femtosecond laser locks the flap back into place, acting as a natural bandage, and your eye heals rapidly. The reshaped cornea allows patients to experience sharper vision almost immediately.

Steps Involved in Bladeless LASIK Surgery:

Using WaveLight FS200 Femtosecond Laser, a laser is applied at predefined depth on cornea forming a canal, a cleavage plane and a side cut to lift the flap.

Flap of precise thickness and shape is formed and now eye is ready for the reshaping to be done with the excimer laser.

Using an Excimer laser the cornea is reshaped permanently to correct the patient’s refractive error.

After correcting the cornea the flap is gently placed back.

LASIK SURGERY PROCEDURE

The surgeon creates a corneal flap, which is a thin slice of the cornea. It is cut and flipped open like the cover of a BOOK. There are two methods of cutting the flap, using microkeratome, which involves a blade, or femtosecond lasers, which are bladeless.

Once the flap has been cut and opened, an excimer laser is used to remove tissue from the centre of the cornea to re-shape it, correcting the patient’s refractive error.

What's the difference between Traditional Lasik & FemtoSecond Lasik?

When Lasik was initially introduced, an instrument known as a Microkeratome was used to cut a thin, hinged flap into the eye’s clear surface (cornea). The flap is lifted for application of laser energy that reshapes the eye for vision correction. But the metal blades were found to be associated with some cases of incomplete or improperly formed ‘buttonhole’ flaps that could lead to vision-threatening scars. Metal blades could lead to uneven flap edges, resulting in abnormal corneal surfaces and vision defects such as irregular astigmatism. Hence to eliminate that threat, Femtosecond Lasers have now replaced mechanical cutting tools (Microkeratome) in conventional LASIK with laser and the procedure is called Blade-free or all-laser LASIK or FemtoSecond Lasik.

How To GO BLADELESS

Nearly all Femto Second Lasik patients can expect to achieve 20/40 vision with Lasik surgery, which is good enough to enable you to drive without having to wear contacts or glasses. Many can expect to achieve 20/20 vision or better. It offers many benefits and should significantly improve your overall quality of life. However, there are no guarantees that you will have perfect vision. Outcome may vary depending on your vision problem and history, age and number of other factors. Patients with high initial correction might still require reading glasses or a thin pair of glasses for activities such as NIGHTdriving. We advise you to look at Lasik not as an option to perfect eyesight, but as a procedure that reduces your dependence on glasses or contact lenses.

LASIK can be used to correct nearsightedness (myopia) up to -12 diopters, farsightedness (hyperopia) up to +5 diopters, and astigmatism up to -5 diopters.

The basic criteria for FemtoSecond Lasik surgery are:

  • You should be 18 years old
  • You should be in good general health
  • You should have a stable vision prescription for at least six months
  • You should have no existing eye disease
  • You should not have severe diabetic eye disease or severe glaucoma
  • You should not be pregnant or nursing
  • You should have cornea of adequate thickness

steps to keep in mind?

Pre op care steps you need to follow include:

  • Stop wearing contact lenses prior to the surgery at least 5 days
  • No perfume, makeup or creams to be applied
  • Eat a light meal before the procedure
  • Arrange for transportation , patient should not drive after the procedure
  • Patient must be accompanied by responsible adult on the day of the surgery

Following the Lasik procedure, patient will experience few temporary complains, which these usually last a short period of time :

  • Foreign body sensation, heaviness
  • Severe watering
  • Light sensitivity
  • Blurry vision
  • Red spot on the white part of the eye

COMMON FAQs

Being completely bladeless, the patient does not experience any pain during the FemtoSecond Lasik surgery. It is the safest and one of the most advanced surgeries in the world. The patient only experiences a slight pressure sensation. The procedure itself is very comfortable for the patient.

Yes, both eyes can be operated upon on the same day.

The Femtosecond Laser and the Excimer Laser takes only a few seconds to create a flap and reshape the cornea. The total time into and out of the operation theatre will take approximately just 10-15 minutes.

Most of the eye growth takes place upto the age of 18 years. After this the eye number tends to stabilize and not change. Lasik should be done after the stability of the glass number has occurred for at least 1 year. The results are then permanent.
